A technology writer has uncovered the original patent filing for rolled toilet paper. And in the patent filing, there's an illustration of how the roll would hang off of a holder.

So there you have it, according to the original 1891 patent, the inventor, Seth Wheeler, intended for the roll to go over, not under.
