Shane Wood, 48, was indicted in Freehold in connection with work performed at a home in Monmouth Beach. He's charged with fourth-degrree counts of forgery, uttering a forged instrument, failure to register with the New Jersey Division of Consumer Affairs and failure to provide a written contract.

According to information from acting Monmouth County Prosecutor Chris Grammiccioni, Wood sought to subcontract the job last January, as principal of Reliable Roofing and Siding.

Investigators say that the general contractor required a certificate of liability insurance to indicate coverage under New Jersey Workers' Compensation Insurance. They contend that Wood submitted verification of coverage by the John Hill Insurance Company.

Representatives of the insurance firm told the contractor that the document was a fraud, prompting a call to the Superstorm Sandy Fraud Task Force.

Wood was released after posting $10,000 bail.
